1. Purpose of Personal Information Use 

The head of the relevant administering agency receiving this application intends to use personal information for the purpose of selecting and verifying eligible beneficiaries, in accordance with Articles 7 and 19 of the Act on the Use and Provision of Social Security Benefits and the Discovery of Eligible Beneficiaries.


2. Scope of Personal Information to Be Used and Consent Requested

I agree to regularly requesting from relevant organizations or searching and applying through relevant information and communications networks (including shared use of administrative information) data or information on personal information and family relationship verification, income, property, work ability, and employment status, social security benefit history, and other information necessary to select beneficiaries, such as resident registration computerized information, family relationship registration computerized information (for basic livelihood security, information on self, spouse, direct ascendants, descendants, and their spouses; for infants and single-parent families, information on self, spouse, and direct descendants; for the elderly and the disabled, information on self and spouse), finance, national tax, local tax, land, building, health insurance, national pension, employment insurance, industrial accident compensation insurance, immigration, military service, veterans' benefits, correction, etc.


3. Retention and Disposal of Personal Information

In accordance with Article 34 of the same Act, personal information will be retained for a period of five (5) years

(However, social security information necessary for the protection of eligible beneficiaries may be retained for more than five years).

Upon expiration of the retention period, the personal information will be destroyed.
